Memorial Wall Plaques
To place an order for a memorial wall plaque:
○ Payment of $298.00 per plaque, and
○ Submit the original and yellow copy of the application form (available at city hall or the chamber of commerce) and mail to:

A memorial plaque must be in the name of a deceased individual to maintain the memorial image. All plaques are done in the same format.
The city will place the order for your plaque as soon as it is received at Depoe Bay City Hall. Orders usually take about six weeks.
